New Member Enrollment Package
For new members, complete the following:
- One Copy is to be Submitted to the DSO-HR as described below.
- Keep the Second copy yourself as a backup in case the application needs to be re-submitted.
- This is a "best practice". Only one is required to be submitted; but it never hurts to have a backup!
- "Open-book" examination.
- Refer to the New Member Student Study Guide, the Auxiliary Manual (COMDTINST M16790.Series) and/or the New Member Reference Guide.
- Examination has a three (3) hour time limit.
- Note that the Test Sheet should not be included in the package you submit.
- Use of the online form is strongly encouraged to reduce the number of potential errors.
- Enrollment Package must have original signatures. Do not sign any applications until you have met with your prospective flotilla commander.
- All signatures must be in BLUE ink.
- Include Proof of Citizenship (either Birth Certificate or Passport).
- Copy of Safe Boating Course Certificate if applicable
- Copy of the DD214 if the applicant served in the Armed Forces

Fill out the Consent to Membership form from the Coast Guard Auxiliary Association, Inc.
Mail enrollment package to the DSO-HR for review.
